FOR THE UTAH NETWORK
Step 1: In order to use the Utah files, the user must first determine the provider's fee schedule (MedXX) classification.
Step 2: Open the provider JSON file and locate the provider and determine the applicable MedXX classification (contract ID 01 = office visit and contract ID 02 = outpatient services).
Step 3: Locate the applicable CPT code on the fee schedule JSON file and use the appropriate contract ID to determine the PPO allowable.
